HOW DO I...
REGISTER AS A NEW PATIENT?

If you live in our practice area and would like to register with us, please complete one of our registration forms that are available from reception. You have the right to express a preference of practitioner. If you wish to exercise this right, please inform our reception staff who will ensure that this information is included in your registration details. Do bear in mind, however, that you will be registering with the practice rather than an individual GP. A practice questionnaire is also required to be completed so that we have some basic knowledge about you whilst we await the transfer of your medical records.

You will be invited to make an appointment for a new healthcare check with a member of the nursing team. This is important as it enables us to record full details of your medical history, take up-to-date blood pressure readings and discuss your general health with you. You will be asked for a urine sample for testing as well as information about any family history of illness and chronic conditions.

Children under five years of age do not need to have a medical but parents will be asked for details of all immunisation dates.



New Baby?

When a new birth is registered with the registrar you will no longer be given a pink registration form to bring into the surgery to register your baby. It is therefore very important that you come along as soon as possible to get your baby registered with a GP. It is essential that this registration is done before your baby has their first appointment at the clinic.
